This is the first step in the application process and we ask that all community leaders complete the following instructions to be considered for any of our leadership programs:
- Download and review the Community Application Meeting Guide and 9 Dimensions of Healthy Community Workbook.
- Meet with a diverse group of people from across the 9 Dimensions of your community to discuss and share responses to the application questions outlined in the meeting guide.
- Complete the Community Application.
- Once these steps are complete, we will confirm your submission and follow up with your designated contact to explain how the selection process will move forward.
